2020/05/29 行動履歴及び健康観察の記録のお願い(2020.05.29)
新型コロナウイルス感染症を予防するために、J-PARCセンターを来訪される方には、
来所前2週間の行動履歴と健康観察の記録をお願いすることとなりました。
以下の様式を参考に、記録し保管して下さるようお願いします。 記録自体を提出することは求めませんが、感染症予防のために必要がある場合には、記録内容についてお尋ねすることもあります。ご協力のほどよろしくお願いします。

なおご自身の行動履歴・健康観察に関して少しでも懸念がある場合には、来訪される前に、J-PARCセンターの担当者（＊）に連絡を入れ、入構の是非などについてご相談ください。
〔例：症状はあったが、普通の風邪と診断された。職場の同僚に、陰性ではあったがPCR検査を受検した人がいる。 等々〕。
（＊）担当者：装置責任者や受入担当者等。連絡先が不明な場合はユーザーズオフィス（029-284-3398）まで
また行動履歴・健康観察記録に基づいて、体調チェックシートを入構時にご提出ください。

2020/05/29 Please record Behavioral History and Health Observation(updated on 29 May 2020)
In order to prevent the spread of the Coronavirus infection disease (COVID-19), J-PARC Center request all users who will visit our facility the following actions.
- Record a behavioral history and a health observation at latest 2 weeks before visit-site.
- Fill in and keep by yourself as we might request for submission.

Please contact the person in charge of your activity/experiment in advance, if you have any concerns about behavioral history and health observation. (E.g. I had symptoms, but Doctor diagnosed as a cold. My colleague had PCR test and negative.)
In case not sure the person simply contact Users Office (Phone 029-284-3398).
Please fill out a health questionnaire when you pick up User ID's.
